HOW TO ENTER
Beginning approximately May 1, 2013, (“Contest Starting Date”) while supplies last, purchase a specially marked package of Jamieson supplements, and look inside the bottle to find a wrapped game card with a unique alpha-numeric code printed thereon, as well as a coupon redeemable on your next purchase of a Jamieson supplement. To enter and participate in the Contest, visit Jamieson’s Flip the Lid website at www.flipthelid.com, click on the “Enter” link, and complete and submit the online registration, including your full name, e-mail address, mailing address and unique alpha-numeric code to confirm your entry into the contest. Please also confirm that you have read and agree to be bound by the Contest Rules, and indicate ‘yes’ or ‘no’ as to whether you wish to opt-in to receive additional information from the Sponsor. Enter as often as you wish. To be eligible, all entries, including the alpha-numeric code, must be received on or before 11:59 p.m. on October 31, 2013, (the “Contest Closing Date”).
NO PURCHASE NECESSARY
To participate without purchase, print your name, e-mail address, mailing address and a telephone number clearly and legibly on a plain piece of paper, and describe in a minimum 50-word, handwritten essay what brand of vitamins you purchase and why. Mail it to: Jamieson "FLIP THE LID"® Contest, No Purchase Request, P.O. Box 157, Toronto Postal Station B, Toronto, ON, M5T 2T3. Limit of one (1) no purchase entry request per household during the contest period. Duplicate essays or requests will be void and discarded. To be eligible to receive a game card in the mail with a unique alpha-numeric code printed thereon, your no purchase mail-in request must be received on or before September 9, 2013, and will become the property of Jamieson.
IF YOU DO NOT HAVE ACCESS TO THE INTERNET
If you do not have access to the Internet and still want to participate, after finding a game card inside a specially marked package of Jamieson supplements or receiving a game card pursuant to your no purchase entry request, keep a photocopy of such game card or record the alpha-numeric code printed on such game card on a separate piece of paper for your reference, and mail the original game card (registered mail is recommended), along with your name, mailing address and daytime telephone number, to Jamieson "FLIP THE LID"® Contest, Code Verification Request, P.O. Box 157, Toronto Postal Station B, Toronto, ON, M5T 2T3. The independent contest organization will enter your alpha-numeric code on your behalf and will contact you after the Contest Close Date only if it is a potential winner. Jamieson Laboratories is not responsible for any entries or no-purchase-mail-in requests that are damaged, destroyed, lost, late or misdirected. To be eligible, all entries with Code Verification requests must be received by October 31, 2013, Contest Closing Date.
To enter and be eligible to win, an entrant must be a resident of Canada who has reached the age of majority in his/her province of residence and who is not an employee of Jamieson, or a member of the immediate family of, or domiciled with, an employee of Jamieson, its subsidiaries, parent or affiliated companies, their respective advertising or promotional agencies, or the independent contest organization. Proof of identification and age must be provided upon request. In these Contest Rules, “immediate family” means spouse, mother, father, sisters, brothers, sons and daughters.
There is one (1) Grand Prize available to be won consisting of One Million Dollars ($1,000,000). The Grand Prize shall be comprised of thirty (30) equal instalments of $33,333.33 CAD each payable annually without interest over a period of thirty (30) years through an annuity purchased in the winner’s name, commencing with the initial payment on December 16, 2013, with subsequent payments to be made on December 16 in each year thereafter with the final amount to be paid on December 16, 2042. Should December 16th fall on a weekend, the payment will be made on the next business day. Instalments will be paid in the form of a cheque.
The odds of winning the Grand Prize are approximately [1:796,107]. The potential winner will be contacted after the Contest Close Date and before November 14, 2013, by telephone. In the event that there is no Grand Prize winner, a random draw will take place on Thursday, November 28, 2013, at approximately 10 a.m. (EST) in Toronto, Ontario, from among all eligible entries received, to select a potential winner of one (1) prize of $10,000.00 CAD paid in the form of a cheque.
HOW TO CLAIM THE GRAND PRIZE
The potential winner will be contacted after the Contest Close Date and before November 14, 2013, by telephone. If you are notified by the independent contest organization that your alpha-numeric code on your original game card is a potential winner of the Grand Prize, make a photocopy of your game card, retain it for your records, and send the original game card with the “winning” code (registered mail is recommended) along with your name, address and daytime telephone number to: Jamieson "FLIP THE LID"® Contest, P.O. Box 157, Toronto Postal Station B, Toronto, ON, M5T 2T3. To be eligible to win, the original game card with the winning code must be received on or before November 28, 2013. Entrants who have already sent in their original game card as part of their Code Verification request do not need to do anything further, but will only be notified if the alpha-numeric code on such original game card is a potential winner.
Upon receipt of the original game card, the independent contest organization will assess and validate such game card after the Contest Close Date. Mechanically reproduced entries will not be eligible or accepted and will be deemed to be null and void. Fraud-detection devices may be placed on the participating game cards. Any game card that has been forged, mechanically reproduced, altered, defaced, mutilated or tampered with in any way, or which contains printing or other errors, or which has been obtained through unintended or unauthorized channels is void. No more than one (1) Grand Prize will be awarded notwithstanding any printing or other errors. By entering the Contest, entrants agree to adhere to the Contest Rules and the decisions of the independent contest organization which are final and binding.
Before being declared a winner of the Grand Prize, the potential winner will be required to first correctly answer a mathematical skill-testing question to be administered by phone or by mail, and to sign a Declaration and Release.
The winner will be required to sign a Declaration of Compliance with the Contest Rules and a Publicity/Liability Release, confirming compliance with the contest rules, acceptance of the prize as awarded, releasing the Sponsor, its affiliates, subsidiaries, parent and related companies, advertising and promotional agencies, the independent contest organization and all of their respective directors, officers, owners, partners, employees, participating retailers, agents, servants, representatives, successors and assigns (collectively, the “Releasees”) from any actions, cause of action, suits, debts, claims, demands and liability whatsoever in connection with this Contest or the acceptance and/or use of any prize, including, without limitation, claims based on negligence, breach of contract and fundamental breach and liability for physical injury, death or property damaged which any entrant, his/her administrators, heirs, successors or assigns has or might have by reasons of, or arising out of, the entrant's participation in this Contest and/or in connection with the acceptance and/or use of a prize. Declaration and Release documents must be returned within ten (10) business days of the date of the notification. If the requisite documents are not returned within this 10-day period, then the prize may be forfeited at the discretion of the Sponsor. Only when all the documents have been completed by a potential winner and verified by the Sponsor will a potential winner be officially declared a winner and receive a prize. By entering the Contest and accepting a prize, the winner consents to the use of his/her name, address, voice, statement and photograph or other likeness, without further compensation, in any publicity or advertisement carried out by or on behalf of the Sponsor in any manner whatsoever including print, broadcast or via the Internet.
By entering the Contest, each entrant agrees to abide by the Contest Rules and the decisions of the independent contest organization, which shall be final and binding, without right of appeal, in respect to all matters related to this Contest, including, without limitation as to eligibility and/or disqualification of entries. In the event it is determined that an entrant has entered in a manner that is not sanctioned by these Contest Rules, and/or has submitted more than the number of entries permitted by these Contest Rules, or has otherwise not complied with these Contest Rules, such entrant shall be disqualified. All entries and requests for no purchase entries and Code Verification requests become the property of the Sponsor, which assumes no responsibility for incomplete, illegible, damaged, destroyed, lost, stolen, falsified, late, or misdirected or, insufficient postage entries or requests for entries, all of which shall be treated as void. Furthermore, the Sponsor shall not be responsible for problems, errors or negligence that may arise or occur in connection with the Contest, including, without limitation, inaccurate, or incomplete capture of entry information or any failure of the website during the promotional period, including, without limitation, as a consequence of any problems or technical malfunction of any kind including, without limitation, in respect to computer equipment, hardware, software, human or technical errors, printing errors, lost, deleted, failed, incomplete or garbled date, communications or transmission interruptions, deletions, defects or failure of any telephone or computer lines or networks, computer online systems, servers, access providers, computer equipment, software, failure of any e-mail or entry to be received by the Sponsor on account of technical problems or traffic congestion on the Internet or at any website, or any combination thereof, including, without limitation, any injury or damage to an entrant’s or any other person’s computer related to or resulting from playing or downloading any material in the promotion, or errors in advertising, the selection and announcement of winners and/or the awarding of any of the prizes.
The Sponsor reserves the right, in its sole discretion, to cancel or suspend this Contest should a virus, bug or other cause, beyond the reasonable control of the Sponsor, corrupt the security or proper administration of the Contest. Any attempt to deliberately damage any website or to undermine the legitimate operation of this promotion is a violation of criminal and civil laws, and should such an attempt be made, the Sponsor reserves the right to seek remedies and damages to the fullest extent permitted by law, including criminal prosecution. Entries are subject to verification and will be declared invalid if they are illegible, irregular, mechanically reproduced, mutilated, forged, falsified, altered or tampered with in any way or that have been submitted through illicit means or do not conform with or satisfy any or all conditions of these Contest Rules.
In the event of a dispute as to the identity of a winner based on an email address, the winning entry will be deemed to be made by the authorized account holder of the email address at the time of entry. Authorized account holder is the natural person who was assigned an email address by an Internet access provider, online service provider or other organization responsible for assigning email addresses for the domain name associated with the submitted email address.
All entries, and requests for no purchase entries, become the property of the Sponsor and none will be returned. No communication will be entered into except with a potential winner.
FOR QUEBEC RESIDENTS
Any litigation respecting the conduct or the organization of a publicity contest may be submitted to the Régie des alcools, des courses et des jeux for a ruling. Any litigation respecting the awarding of a prize may be submitted to the Régie des alcools, des courses et des jeux only for the purpose of helping the parties reach a settlement.
